After scheduling your Telehealth appointment, please complete all patient forms, and send them along with lab results, and other pertinent information, to Dr. Werner through Clocktree's Secure Messaging.  

Prior to your appointment, please submit full payment below.  

If Dr. Werner is a provider for your insurance carrier, please submit only your copay.  If your insurance policy does not cover the doctor’s Telehealth services, you will be responsible for the full amount.
