Our office no longer accepts insurances. Each patient will receive a “super bill” that they may submit to their insurance carriers for possible reimbursement of fees. Please note that our office will not submit claims to insurance companies nor will we check patient coverage. It is the patient’s sole responsibility to determine if our-of-network services are covered by their policies.

After scheduling your appointment, you will receive an email from our office with further instructions and a link to our hipaa-secured patient forms.
